Single Vial Kit Instructions
- To open collection cup, tear shrink banding along perforations, which are marked with printed arrows.
- Remove vials and bag from collection cup.
- Top of collection cup can be used in collection of the specimen.
- Tear Shrink Band along perforations on vial
- Snap vial closed completely.
- Fold lid tab down to lock on vial.
- Place Custody and Control form seals over vial.
- Insert vial(s) into front pocket of Specimen bag.
- Take completed copy of Custody and Control form and insert into rear pocket.
- Remove release liner from bag and seal bag.
