Single Vial Kit Instructions

1. To open collection cup, tear shrink banding along perforations, which are marked with printed arrows.
   
2. Remove vials and bag from collection cup.
   
3. Top of collection cup can be used in collection of the specimen.
   
4. Tear Shrink Band along perforations on vial
   
5. Snap vial closed completely.
   
6. Fold lid tab down to lock on vial.
   
7. Place Custody and Control form seals over vial.
   
8. Insert vial(s) into front pocket of Specimen bag.
   
9. Take completed copy of Custody and Control form and insert into rear pocket.
   
10. Remove release liner from bag and seal bag.
